Supportive Case Management provides services to adults (age 18 and over) who meet the New York State Office of Mental Health's criteria for severe and persistent mental illness.
The Neighborhood Center, Inc., works in partnership with Catholic Charities and Family Services of the Mohawk Valley to provide this service from three separate locations (two Utica-based locations and one site in Rome) to better serve the constituency.
Many people diagnosed with mental illnesses can be treated and/or managed successfully on an outpatient basis with medications and therapy, but these individuals may still need professional assistance confronting and overcoming obstacles and barriers to independent living. Supportive Case Managers help "level the playing field" by providing care coordination services including advice, services and support to clients with regards to such needs as obtaining clinical and treatment services, medication management assistance, help finding and securing adequate housing, and obtaining training and employment. Staff also help with day-to-day needs such as shopping and budgeting and many perform a variety of advocacy services as well.
