JOB DESCRIPTION: The Care Manager is part of the Integrated Treatment Program (ITP) multidisciplinary team and is responsible for assisting individuals in outpatient treatment for dually diagnosed mental health and substance use disorders with complex needs. The role of the Care Manager is to promote recovery by providing person-centered, culturally informed care coordination.
SPECIFIC RESPONSIBILITIES INCLUDE:
- Works directly with clients to complete assessments and assist them with identifying individual goals and establishing a care plan.
- Provides care coordination, advocacy, and support, for an assigned caseload to assist clients in achieving their identified goals.
- Encourages clients to engage in treatment and services, and provides assistance to address barriers as needed.
- Assists clients with the maintenance or attainment of benefits/entitlements as needed.
- Assists clients by arranging appointments with service providers and facilitating the receipt of services as needed.
- Responds to crisis situations and links clients to Crisis Services per policy.
- Adheres to the guidelines and procedures as established by regulatory bodies including the Office of Mental Health (OMH), Department of Health (DOH) and Medicaid.
- Maintains self in professional manner at all times; adhering to The Neighborhood Center, Inc. policies, procedures, and in keeping with the Agency’s mission statement.
- Performs other duties as requested by supervisory staff or agency administrators.
REQUIRED QUALIFICATIONS:
- Knowledge and experience with mental health and substance use disorder service delivery systems and community resources.
- Ability to foster effective, professional partnerships with diverse populations and be committed to supporting and empowering recovery.
- Ability to maintain satisfactory expectations for timeliness and accuracy in the performance of the job functions.
- Ability to work independently with good organization and time management skills.
- Knowledge of community resources.
- Computer skills including the use of email, Word, Excel, and the Electronic Health Record (EHR).
- Ability to work in a variety of settings including the clinic, clients’ homes, hospitals, and other community settings.
- Possesses a current and valid New York State Driver’s License and has reliable transportation.
- A Bachelor’s Degree in Social Work, Nursing, Psychology, or a closely related Human Service field and two (2) years of related experience in the Mental Health/Addictions field are required.
