CASE MANAGER– KINSHIP CONNECTIONS PROGRAM

Full-time position with an excellent benefits package. Be a part of a team providing crucial services to children and their caregivers living in a kinship family situation.

Basic work schedule is 8:30am – 4:30pm: Monday thru’ Friday with flexibility needed to meet program/service needs including working some weekend and evening hours as necessary. The Kinship Program serves Oneida and Herkimer Counties in New York State.

JOB DESCRIPTION: The Kinship Connections Case Manager will work with children and caregivers living in a kinship family situation. The Case Manager will act as resource and advocate for families within Oneida and Herkimer counties and will work with families in accordance with their needs following best practice for trauma informed care.

SPECIFIC RESPONSIBILITIES INCLUDE:

• Conduct (minimally) monthly home visits.
• Assess for trauma and family functioning, provide appropriate follow up.
• Develop and implement individual service plans.
• Provide supportive information to caregivers and families.
• Provide or facilitate crisis intervention.
• Conduct community outreach.
• Arrange or conduct caregiver and child support groups, family engagement activities. community education activities; short-term respite activities as needed.
• Maintain appropriate documentation of work and maintenance of family record.
• Assist children and caregivers in the development of skills and access appropriate. resources in a manner designed to enhance healthy family interactions and provide a safe and nurturing home for children.
• Assist families in accessing Non-Parent Caregiver grants, child support, Supplemental Nutrition Assistance Program (SNAP), KinGap and other financial resources as well as medical, legal, and educational services.
• Fulfill training requirements of all funding and regulatory bodies.
• Assist supervisor in assessing the need for supplies, materials, equipment and repairs.
• Maintain a program environment that is safe, healthy, orderly and attractive.
• Maintains self in a professional manner at all times and adheres to the policies and procedures of the Agency.

• Performs related tasks as assigned or required.

REQUIRED QUALIFICATIONS:

• Bachelor’s Degree in Social Work, Psychology, or Human Services required

OR

• Bachelor’s Degree in any field and a minimum of two (2) years of Case Management experience required. EOE
