Psychosocial Rehabilitation Service Provider- FT


FULL-TIME position with excellent benefits is available in our Children and Family Treatment and Support Services (CFTSS) program. This program with The Neighborhood Center, Inc. is growing!

Goal: These clients may have difficulty engaging in formal office settings but can benefit from home and community-based rehabilitative services.

Responsibilities include but are not limited to:
• Restore and rehabilitate a child’s developmentally appropriate functioning as necessary for the integration of the child as an active and productive member of her/his family and community.
• Valid NYS Driver’s License and transportation required. Travel to homes and within the community is required. Non-traditional hours such as evenings and weekends will be required to accommodate family needs and children’s school schedules.

The services may include:
• Promote personal and community competence, promotes personal independence, autonomy and mutual supports by developing and strengthening the individual’s independent community living skills and integration into the community.
• Build and enhance social and interpersonal skills (to increase community tenure and avoid more restrictive placements)
• Build and enhance personal relationships.
• Establish support networks.
• Increase community awareness.
• Develop coping strategies.
• Build and enhance daily living skills to improve self-management of the negative effects of psychiatric, emotional, physical health, developmental or substance use symptoms that interfere with a person’s daily living.
• Promote personal autonomy skills such as learning to manage stress, unexpected daily events and disruptions, behavioral health and physical health symptoms with confidence.
• Community integration includes implementing learned skills so the child can remain in her/his natural community location and achieve developmentally appropriate functions in the following areas: Social Skills, Health Skills, Effectively responding to or avoiding identified precursors or triggers that result in functional impairments.

• Staff must be at least 18 years of age.
• Must have a High School Diploma/Equivalency and have a minimum of 3 years of experience in children’s mental health service, addiction and/or foster care services. EOE
