POSITION: Schoharie Otsego Herkimer Oneida (SOHO) Regional Permanency Resource Center Care Manager (RPRC)
- FULL TIME POSITION with excellent benefit package.
- 37.5 hours per week
- Monday – Friday ~ 8:30-4:30 with occasional flexibility needed for evening events.
JOB DESCRIPTION: The SOHO Regional Permanency Resource Center Care Manager will work with children and caregivers living in post-guardianship or post-adoptive families following the Illinois Adoption and Guardianship Preservation Program model. The Care Manager will act as resource/advocate for families within Schoharie, Otsego, Herkimer and Oneida counties and provide information to families in surrounding counties. The Care Manager will ensure children are safe and maintained in their adoptive homes or homes of their guardians by ensuring families have the tools they need to provide for the social/emotional development and mental health needs of the children and family members reducing the impact of trauma on children and families, addressing identified challenges, exploring individual perceptions that could result in other issues and concerns, identifying resources and supports, and building stronger connections within the family.
SPECIFIC RESPONSIBILITIES INCLUDE:
- Provide families with the supports they need to care for their children; help maintain the stability of the placement, reduce the need for higher levels of care during crisis situations; and meet program outcomes;
- Provide on-going assessment using program tools, follow-up and enhancement of services which promote safety and general well-being for at-risk children, adolescents, families:
- Conduct (minimally) bi-monthly home visits and develop a Plan of Care in collaboration with the family;
- Provide service strategies, approaches, and linguistic capacities that promote the delivery of services that are culturally competent and reflective of the families and children to be served; refer, facilitate access and accompany caregiver to medical, legal and educational services;
- Promote cross-agency dialogue and partnership regarding service planning;
- Assist families in accessing financial resources such as Non-Parent Caregiver grants, child support, SNAP, KinGap;
- Provide individual and group parenting/child development classes using program curriculum and tools.
- Develop and implement strategies to engage youth in programs and services;
- Maintain data and case files in accordance with performance standards/outcome measures;
- Fulfill training requirements of all funding and regulatory bodies;
- Participate in program recruitment and outreach in the community;
- Assist supervisor in assessing the need for supplies, materials, equipment and repairs;
- Maintain a program environment that is safe, healthy, orderly and attractive;
- Maintains self in a professional manner at all times and adheres to the policies and procedures of the Agency.
- Performs various related tasks as assigned or required.
REQUIRED QUALIFICATIONS:
- Bachelor’s Degree in Social Work, Human Services, Education or closely related field required with related experience preferred. Residency in or familiarity of the area to be served is preferred.
