JOB DESCRIPTION: The Community Health Worker (CHW) will work with individuals and families to access needed preventive and primary health care services with an emphasis on pregnant women not enrolled in health care or supportive services. The CHW will assist with implementation of the Community Health Worker strategies in coordination with Mohawk Valley Perinatal Network in support of the PICHC project.
Under the supervision of the Community Health Worker Program Supervisor, the CHW duties and responsibilities include:

  • Community Health Worker position will require Home Visits.
  • Identify and assist individuals and families to access needed preventive and primary health care services (e.g. preconception care, family planning, prenatal care, immunization, pediatric care and care for acute, chronic, and communicable diseases, Early Intervention, Women, Infants and Children (WIC) and other nutrition services, inter-conception care with an emphasis on pregnant women not enrolled in health care or supportive services.
  • Conduct basic health assessments, assists families to identify needs, provides basic health information, and make appropriate referrals through monthly home visits.
  • Provide assistance and/or referrals to obtain other essential support services such as housing, financial aid, food stamps, emergency food, clothing, transportation, translation, and child care.
  • Provide advocacy, support and follow-up to determine if services are received and assist families with health behavior changes.
  • Provide home visiting services to approximately 25 clients at any given time plus an additional 10-15 intermittent clients (clients who do not need intensive case management, but referrals to a community resource or service). Program goal is 40 annually.
  • Participate in program recruitment and outreach in the community.
  • Provide supervisor with input/information as to special needs participants may have, problems, incidents and emergencies to ensure proper action/intervention is taken
  • Maintain accurate records of statistical information, records, summaries and referrals, family plans and goals and any other information as required.
  • Attend staff or other types of meetings as required.
  • Fulfill training requirements of all funding and regulatory bodies. Must attend training provided by the New York State Department of Health (NYSDOH) Maternal and Infant Health –Center of Excellence.
  • Serve as a role model.
  • Assist supervisor in assessing the need for supplies, materials, equipment and repairs.
  • Maintain a program environment that is safe, healthy, orderly and attractive.
  • Participate in various related tasks as assigned or required.

REQUIRED QUALIFICATIONS:

  • Community resident of the Oneida or Herkimer County with particular focus on Utica, Rome, Taberg, Blossvale, Boonville and Camden in Oneida County and Frankfort, Ilion, Herkimer, Little Falls, Mohawk, Ohio, Newport and Poland in Herkimer County.
  • Writing ability sufficient to provide adequate documentation in the family record, referral forms and other service coordination forms, and reading ability to the level necessary to comprehend training materials and assist others to fill out forms.
  • Knowledge of the community, community organizations, and community leaders.
  • Ability to work flexible hours, including evening and weekend hours.
  • High School Diploma/General Equivalency Diploma with related experience required.
  • Valid Drivers License

Preferred (not required):

  • Bilingual skills, depending on the community and families being served.
