Care Management

The Care Management Program helps to connect people with the services they need: housing, primary care doctor, mental health services, substance use disorder treatment, dentist, support for applying for public assistance programs (such as SNAP, WIC, Temporary Assistance, Social Security Disability (SSD), etc.). Our services connect individuals to social programs, resources for transportation, and other specialty providers.

How We Help?

  • Our team of care managers will work in collaboration with your natural supports and other service providers to ensure all needs are being met.
  • Members are empowered to develop goals that they believe will improve their overall health and wellbeing.
  • We offer Care Management services for Adults & Children in Oneida & Herkimer Counties

Children's Care Management

Children's Care Management services Children age 0-21 in Oneida and Herkimer Counties. Care Managers work with the child/youth and family to ensure all the child's biopsychosocial needs are being met. The care managers provides linkage to services including primary care, dentist, behavioral health and social support services. The care manager also works together with service providers to collaborate services and provide advocacy to ensure the child's needs are adequately met.

Adult Care Management services are available to Medicaid and non Medicaid enrollees.
Children's Care Management services are available to Medicaid enrollees.

Herkimer County
Adult Care Management services are available to Medicaid enrollees.
Children's Care Management services are available to Medicaid and non Medicaid enrollees.
