

Do I need to bring my insurance card to every visit?
Yes, it is important for our support staff to see and verify your insurance cards are the same at every visit, to ensure we are billing correctly.


Do I need to bring custody/guardianship paperwork to the in-take for a child?
Yes please make sure to bring any custody paperwork, guardianship paperwork with you to the initial in-take, or if at anytime custody has changed during the course of the child’s treatment.


Does it cost anything to join the program?
No. The program is funded by the Office of Children and Family Services through a five-year grant.


Can anyone be in the program?
No. The program is open to families who have adopted a child or has legal guardianship of a child. Program staff can work with the family no matter how long ago the adoption or guardianship took place-yesterday, three years ago, a teenager adopted as a baby…


Does it cost anything to attend the Family Fun Days?
No, there is no cost for the families to participate in the Family Fun Days.


Do I need to be pregnant to get Community Health Worker services?
No, if you have never been pregnant before, are pregnant now, are postpartum or have children then you are a fit for services.


How will I get to my appointment?
The Community Health Worker program is a free home visiting service.


What can a Community Health Worker do?
Help you find needed health care, resources and services and refer you there (i.e. health insurance, a doctor, a dentist, a counselor, family planning, breastfeeding support, a parenting class and so on).


What else can a Community Health Worker do?
Provide education on health topics such as The Importance of Taking Folic Acid, Family Planning, Oral Health, Prenatal Care, Breastfeeding, Postpartum Depression, Safe Sleep for Babies and much more.
