Position:  Social Care Network Navigator
Program: Care Management
Status: Full Time with Benefits
Location: Utica, NY in person
Rate of Pay: $21.98
Base Schedule: 35-hour work week. Monday – Friday 8:30-4:30.
JOB DESCRIPTION: A Social Care Navigator (SCN) is responsible for assisting eligible Medicaid members through coordination of community resources, and assistance with navigating
support system in order to adequately address health disparities and improve member health. Navigator will be responsible for conducting HRSN screenings, eligibility assessments, and
navigation activities for Medicaid Fee-for-Service and non-eligible Managed Medicaid members.
SPECIFIC RESPONSIBILITIES INCLUDE:
  • Manage incoming referrals for screening and navigation to ensure successful and timely connections for community members.
  • Assign Members already connected to an established contracted care team to their SCN-designated screener and/or Social Care Navigator to conduct screening and navigation activities, fostering continuity with existing contracted care teams.
  • For those Members not engaged, conduct and document outreach in alignment with required frequency, modality, and timeframe, including “hot spotting" in community agencies to build relationships and conduct screenings.
  • Manage Member consent and attestation as required throughout the engagement.
  • Conduct HRSN screening using the Accountable Health Communities (AHC) screening tool to assess member HRSNs.
  • Conduct eligibility assessments to determine Member eligibility for enhanced HRSN services and refer Members to eligible programs and services, including enhanced HRSN services and/or existing federal, state, and local resources
  • Maintain effective communication with internal team members, community members, and partner organizations to ensure acceptance, resolution, or redirection of referral requests.
  • Document progress notes and action taken with each referral as detailed in the Network Standards and Quality Program.
  • Regularly use data and data tools to report referral patterns and trends to the Referral Coordination Manager.
  • Completes professional and timely documentation for all appointments/contact with clients/collateral contacts per program guidelines.
  • Completes all compliance/required training(s) in outlined timeframes including those required to serve Medicaid members either through trainings provided by Healthy Alliance or through other means. Attends weekly/monthly staff meetings.
  • Maintains self in professional manner at all times, adhering to The Neighborhood Center, Inc. policy, procedures and in keeping with the Agency’s mission statement.
  • Punctual and regular attendance is an essential responsibility of each employee.  Employees are expected to report to work as scheduled, on time and prepared to start working.
  • Performs other duties as requested by supervisory staff or agency administrators.

REQUIRED QUALIFICATIONS:

  • Associate degree in health, social services, or related field preferred. Equivalent work experience in a related field may be considered in lieu of degree requirements.
    AND
  • Minimum of 5+ years related experience in a clinical, non-profit, or Managed-Care Organization (MCO) environment preferred.
    AND
  • Extensive knowledge and understanding of health equity, social drivers of health, and social care data.
  • Excellent communication and presentation skills.
  • Experience using translation services preferred.
  • Ability to build collaborative working relationships with others inside and outside the organization through cooperation and mutual respect. Capacity to inspire and motivate others.
  • Thrive working with multiple systems and processes.
  • Demonstrate ability to use various technology platforms to ensure successful and timely referral connections are made.
  • Extremely detail-oriented and capable of multitasking.
  • Proven record of hitting key metrics, defining effective data-driven network development strategies, and problem-solving.
  • Valid New York State Driver’s License and reliable transportation.
