COMMUNITY BASED SERVICES PROGRAM SUPERVISOR

Pay: $31.79 per hour

Shift: Monday – Friday 9:00 a.m. – 5:00 p.m. with flexibility as needed 37.5 hours per week.

POSITION: COMMUNITY BASED SERVICES PROGRAM SUPERVISIOR

JOB DESCRIPTION: Under the supervision of the Program Director, the Community Based Services Program
Supervisor is responsible for the leadership and administrative functions for the Children and Family Treatment
and Support Services working with children with behavioral health needs in the home and community. Responsible
for the direct supervision of CFTSS providers and implementation of the Children and Family Treatment and
Support Services as designed.

SPECIFIC RESPONSIBILITIES INCLUDE:

  • Under the supervision of the Program Director, is accountable for the day-to-day program operations and
    supervision of staff, including completion of employee appraisals.
    ● Plan and coordinate outreach, supervise individual and family assessments, assess training needs and provide
    education, home visiting, and service coordination performed by CFTSS Providers.
    ● Ensure home visits or in-person contacts are made by providersas scheduled according to the treatment plan
    determined by the needs of the family.
    ● Establish relationships with community resources and other human service providers; develop and maintain
    community resources within each county.
    ● Arrange for or conduct support groups for CPST and PSR services as outlined by the treatment plan and/or
    recommended by the LPHA.
    ● Provide leadership, training, supervision, evaluation and professional development opportunities for staff in
    accordance with agency policies and procedures. Must attend training provided by New York State and/or entities
    as deemed appropriate.
    ● Ensure systems are in place to provide effective outcome monitoring and continuous quality improvement in all
    program functions.
    ● Maintain data in accordance with performance standards/outcome measures to demonstrate effectiveness of
    program design, implementation and sustainability as well as entering into the electronic medical record.
    ● Ensure programs and services operate to capacity including quality and quantity of services provided and
    individuals served.
    ● Ensure program compliance with New York State Office of Children and Family Services regulations in addition
    to all other local, state and federal governing laws and regulations.
    ● Ensure utilization of best practices and research evidenced-based practices.
    ● Prepare required reports and other written material regarding program implementation and activities.
    ● Complete monthly, quarterly, yearly and all other required reports accurately and in a timely manner to program director.
  •     Attend various community/county meetings on behalf of the agency.
    ● Ensure that the physical plant in which programs occur is maintained in a safe, healthy, orderly, attractive and
    nurturing manner and is conducive to providing enriching experiences while ensuring program personnel maintain
    agency equipment and consume program supplies responsibly.
    ● Interpret to staff: agency objectives, policies, personnel policies, and evaluation procedures. Ensure that
    orientation materials are up-to-date, distributed to staff in a timely manner, and interpreted clearly.
    ● With input from the Program Director and Division Director, recommend to the Executive Director: applicants
    for hire, staff for additional assignments or promotion, staff for termination.
    ● Ensure appropriate supervision by modeling and positively interpreting agency objectives, policies, and directives
    which include, but are not limited to: conducting periodic evaluations for staff directly supervised by this position,
    and ensuring all staff fulfill training requirements of agency, funding, licensing, and regulatory bodies.
    ● Maintain self in a professional manner at all times and adheres to the policies and procedures of the Agency.
    ● Punctual and regular attendance is an essential responsibility of each employee. Employees are expected to report
    to work as scheduled, on time and prepared to start working.
    ● Employees are expected to be available to work evening and weekends as assigned, based on the needs of the program.

REQUIRED QUALIFICATIONS:

  •     Knowledge of the specific geographic area and the population to be served.
    Bachelor’s Degree in Social Work, Psychology, Human Services, or related field and three years related
       experience with one year supervisory experience strongly preferred.
